Active - $100.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) Active members of the Society shall be persons licensed to practice medicine in the State of Indiana whose full-time professional employment is/was in the field of clinical, administrative, or research neurology in the State of Indiana, who are eligible for membership in the American Medical Association, and who have completed a neurological residency accredited by the American Board of Psychiatry and Neurology.

Affiliate - $85.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) Affiliate members shall be physicians or doctoral level persons whose full-time professional employment is in fields related to neurology (e.g., but not limited to, neurosurgery, neuro-ophthamology, neuro-otology, neuroradiology, neuropathology, psychiatry, physiatry, and PhD’s in research or clinical work). Affiliate members shall have the same benefits as active members with the exceprion of voting rights, being elected to the Executive Board, and participating on Committees. To be elected to membership, Affiliate members must be approved by the Membership Committee of the Society, and will pay predetermined dues.

Associate - $75.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) Associate members shall be Advanced Practice Providers who are certified as Nurse Practitioners and Physician Assistants and are actively participating in the specialty of Neurology. These persons shall have provided written documentation, from the Neurologist with whom they collaborate, of their continuous participation in the specialty of Neurology for the previous 3 years. They will continue to be required to provide written documentation annually, from the Neurologist with whom they collaborate, endorsing their participation in the specialty of Neurology concurrent with their request for membership renewal. Such Associate members will enjoy the same privileges as Active members, with the exception of being elected to the Executive Board, subject to the payment of annual membership dues.

Associate (Provisional) - $75.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) Provisional Associate members shall be Advanced Practice Providers who are certified as Nurse Practitioners and Physician Assistants and are actively participating in the specialty of Neurology but for less than 3 continuous years. They must provide written documentation annually, from the Neurologist with whom they collaborate, endorsing their participation in the specialty of Neurology concurrent with their request for membership and its renewal. They will enjoy the benefits of Associate members with the exception of voting rights and participation on Committees.
